In an effort to make improvements to the site and better serve you, I’ve but together a brief survey. The survey is short and should take you only about 5 minutes to answer. I’d appreciate if you would respond as truthfully and accurately as possible.

I just filled out the survey but there is no link for submission. Does anyone know how to submit it?
I took it Jackie…and it was at the bottom of the survey….

Maybe it is my computer – but I can’t find the submit button either
Below the last question (email address) there is a white box that says “Submit”, if for some reason you don’t see the text, just try cling in the box anyway. If that doesn’t work try refreshing the page and see if it will come up. I’ve never had trouble using Google forms before so I’m not sure what the problem is now but I’ll look into it. Sorry for the inconvenience.
The last question that I see is for “Name” and there is no Submit button after that.
