To receive your Reward Voucher, after qualifying purchases have been made, go HERE and print the redemption form. Then mail your completed redemption form along with original receipts of the specially marked participating qualifying products (no copies accepted) to the listed address. All items must be postmarked by 6/30/09. Once your information is received you should receive your beauty treatment voucher by mail within about 4 – 6 weeks. A list of participating locations will be provided on the Rewards Website which will be listed on your Reward Voucher.
